THE ARTS SOCIETY DEVIZES RENEWAL FORM

Please fill in and submit this online form to renew your membership of The Arts Society Devizes.

If you would like your subscription to be gift-aided, then tick the Gift Aid checkbox(es) and we will send a gift-aid form to you. You only need to do this if your address has changed otherwise this will carry on from last year.

On submission you will receive an automatic email acknowledging your renewal request. If it doesn't arrive straightaway please check your spam folder.

Membership fee: £50 per person for the membership "year" October 2024 to July 2025.

Please pay via internet banking if possible to: The Arts Society Devizes, Sort code: 52-30-27, Account number: 35839716. Add your surname as the reference.

If paying by cheque, then send your cheque (made payable to: THE ARTS SOCIETY DEVIZES) to Mrs Judith Page or bring it to the next lecture.

If you have set up payment by Direct Debit then payment will be taken at the end of July.

When your payment has been received, our membership secretary, Mrs Judith Page, will send you an email confirming renewal of your membership.
